Services & Treatment Program

-Low caseloads (10 placements per Case Manager)

-Individualized programming and service plan developed for all placements.

-Coordination of all available and appropriate services on behalf of child.

-Weekly documentation by Case Manager of all home visits, contacts, phone calls, and correspondence made and received in regard to child.

-Transportation to and from court hearings, court reports and counseling summaries are submitted prior to each court review.

-Transportation to and from family visitation (per initial agreement with OFC and depending on distance involved.)

-Replacement clothing at no cost to OFC or specialized foster parents.

-Keep a “Lifebook” for child in placement.

-24 hour a day availability to foster parents, child in placement, and other agencies, 7 days per week.

-Availability for case reviews as required by SDFC.

-Provide respite care as needed.

-Extensive support, ongoing monthly training, and yearly all-day workshop for specialized foster parents and Debra Corn Agency, Inc. staff.

-Weekly consults between Debra Corn Agency, Inc. Case Manager and Supervisor to discuss child and foster family. Regular staffing of each child.

-Monthly meeting between Counselor and Debra Corn Agency, Inc. Case Manager to discuss child.
